Purpose
This form records permission for Abryx to send connection-request SMS notifications to the phone number provided below.
SMS permission is separate from approval of any connection request. Receiving a message, opening a secure request link, or reviewing a request does not approve the connection. Each connection request must be separately reviewed and approved or declined by the recipient.
